Contact Us - Short Form with Captcha
for osCommerce Online Merchant v2.2
This is a short contact form with captcha security code included.
Please backup your database and any files that need to be changed - there are only 2 files that
have been changed and 1 new file for this contribution. You must have the GD Library installed with php for this contribution to work.
There are 2 new fields in addition to the original form: phone number and security code
Name, e-mail, phone, and security code are all required. The form maintains the same security code once the user has entered it correctly.

20070806 UPDATE v1.2 (Complete Package)
Updated so that when the security code is entered incorrectly, nothing appears in the security box versus some jumbled text showing there.
Updated ERROR_SECURITY text to cover both blank and valid security code entries.
Updated installation instructions to say replace/update.
